National Consultant for PECP (Lead Poisoning)

A. Objectives
To support MOHFW for mainstreaming environmental health, including lead exposure in existing national programs and support inter-ministerial collaboration
B. Background:
More than half a billion children living in low- and middle-income countries are at risk from elevated lead levels in their blood. Lead is especially toxic to young, growing brains and bodies. In particular, lead can affect children's brain development resulting in reduced intelligence quotient (IQ), attention span, educational attainment, lifetime earnings, and increased antisocial behaviour. Its effects are not always obvious, but they are profound and can be permanent.
The World Health Organization notes that there is no known "safe" blood-lead level. As lead exposure increases, the range and severity of symptoms and effects also increase. According to The Lancet Commission on Pollution and Health, childhood exposures also have been tied to long-term health damage in adulthood, including kidney impairment, hypertension, and cardiovascular disease. Undernourished children are more susceptible to lead because their bodies absorb more lead if other nutrients, such as calcium or iron, are lacking. Children at the highest risk are the very young (including the developing fetus) and the economically disadvantaged. Exposure of pregnant women to high lead level can cause miscarriage, stillbirth, premature birth and low birth weight.
Despite global initiatives addressing lead in paint and gasoline, unsafe smelting of lead-acid batteries and irresponsible uses of lead persist, which is exposing children to contamination on an unimaginably large scale. This lead exposure stems primarily from substandard, informal, and illegal smelting and disposal of used lead-acid batteries (ULAB). Inappropriately recycled lead is also used as an illicit additive in goods exposing children including pottery glaze, food ingredients, and spices. This is unacceptable, untenable and completely preventable.
Globally around 1 in 3 children, up to approximately 800 million, have blood lead levels at or above 5 μg/dL, which is a reference value used by the U.S. CDC to identify children who have been exposed to lead and require case management. In Bangladesh the current mean children's Blood Lead Level is 6.83 µg/dL and some 35 million children are estimated to have blood lead level above 5ug/dl. Children in Bangladesh have been significantly exposed to lead from various sources including Used Lead-Acid Battery (ULAB) recycling and spices industries.
In 2020 UN Agency and Pure Earth has launched Protecting Every Child's Potential (PECP) initiative for reducing lead exposure among children in four countries, Georgia, Ghana, Indonesia and Bangladesh from 2020 for three years. UN Agency Bangladesh will support health and communication sectors. UN Agency will build professional capacity to support catalytic action by engaging stakeholders at national and local levels.
C. Scope of work
The consultant will work closely with DGHS, MOHFW, MOEFCC and UN Agency to mainstream Lead Poisoning in health sector. The consultant will provide technical assistant to DGHS to facilitate national stakeholder group meetings and seminars with experts and program focal persons from various organizations and ministries. The consultant will develop training materials for health service providers based on global tools.
Under the overall supervision of the Health Specialist of Health Section, UN Agency Bangladesh, the Consultant will carry out the following tasks and duties:
- Support to MOHFW for mainstreaming environmental health, including lead exposure in existing national programs and support formation and coordination of Technical Advisory Group
- Coordinate Evidence generation activities for Health Sector Situation analysis on Lead poisoning in Bangladesh, including assessment of institutional capacity, service providers' capacity and awareness among decision makers and communities.
- Establishment of Blood Lead Level surveillance system under PMR
- Coordinate Policy Advocacy with a national seminar to discuss environmental health and Lead Poisoning
- Develop training materials and guideline for health service providers on environmental health, including lead exposure.
- Coordinate the development of communication materials on environmental health and Lead exposure
D. Deliverables and Schedule
As per the country implementation plan, the consultant will provide technical support to DGHS, MOHFW for the below deliverables within the timeline.
- Prepare the list of Technical Advisory Group (TAG) on Lead Poisoning for DGHS and facilitate introductory group meeting
- Endorsement of TAG by DGHS, Facilitate inception meeting for Health Sector Situational analysis on capacity and awareness of Lead poisoning in Bangladesh. Collect feedback and finalize the inception report
- Development of national awareness-raising campaign materials and national IEC materials for health facilities and schools based on Global communication materials; Supplement healthy lifestyle curriculum targeting school-going children with Lead poisoning module
- Facilitate TAG meeting for review of communication and campaign materials - Collect feedback and finalize materials
- Develop Concept note for National Environmental Health Seminar; Develop training materials for health service providers (based on Global tools) on Lead Poisoning and environmental pollution
- TAG meeting for review and finalization of Concept note for Environmental Health Seminar; Endorsement of Communication Campaign materials by MOHFW (month 6)
- Finalization of logistics (venue and invitation, printing) for Environmental Health Seminar; Facilitate TAG meeting for review and feedback on training materials for health service providers.
- Facilitate TAG meeting for review of preliminary findings of Health sector situational analysis. Provide technical assistance for finalization of report
- Provide technical assistance to prepare dissemination materials for environmental health seminar; Facilitation of environmental health seminar
- Finalization of training materials for health service providers on Lead Poisoning and environmental pollution
- Endorsement of training materials for health service providers on Lead Poisoning and environmental pollution

E. Consultancy Costs
Cost of the Consultant:
- The consultant will be home-based with frequent meetings with DGHS, MOHFW and UN Agency.
- The consultants will get BDT 300,000.00 per month for 11 months as a consolidated package which includes lumpsum travel cost in duty station, so consultant will not claim any travel cost in addition to this. If the consultants are required to travel outside their duty station, prior approval will be required, then travel and DSA costs will be reimbursed at actuals.
- UN Agency will not provide consultant with computer/laptops/printers/office space.
- Stationery, communication and other office equipment costs will have to be borne by the Consultant;
- Consultant will be responsible to pay all taxes as per government rules and regulations. E-Zone will make statutory deductions towards income tax as per government rules and regulations.
F. Qualification requirements
- Post-graduate degree in Public Health/Medical Science/Epidemiology or any other related subjects;
- Minimum 8 years of experience in the health sector;
- Working experience in the areas of public health, environmental health and pollution in Bangladesh;
- Experience in capacity building of health service providers, development of training and communication materials for health sector;
- Understanding of public health systems of Bangladesh;
- Understanding of environmental health and heavy metal poisoning;
- Documentation and presentation skill.
- Excellent Communication skills.
- Fluent in English and Bangla.
