A. Objectives:
The purpose of the consultancy is to support the project on "Saving Children with Severe Pneumonia with or without severe malnutrition through Day-Care Approach in Health Systems of Bangladesh" to develop national guidelines and protocols for the implementation of the Day care for children with severe pneumonia with or without severe malnutrition.
The consultants will provide technical assistance to NNHP, NNS & IMCI to develop national guidelines and protocols for the implementation of the Day care for children with severe pneumonia with or without severe malnutrition in coordination with related department under DGHSand other development partners. The consultant will also support NNHP, NNS &IMCI to organize training for skill development of service providers.
B. Background:
Pneumonia is the leading infectious cause of child death. In 2019, pneumonia accounted for more than 800,000 child deaths under the age of five, with more than 2,000 every day. Malnutrition is a major underlying risk factor for developing severe pneumonia & related death in children. Malnutrition is an underlying cause of death of 2.6 million children each year - a third of child deaths globally.
Despite remarkable progress in reducing under-five mortality in recent decades and existence of low-cost and effective interventions, pneumonia remains the leading cause of morbidity and mortality among under five children for more than 3 decades in Bangladesh.
Management of severe pneumonia requires hospitalization for supportive care. Studies demonstrate that an alternative treatment option that has been tested and found to be effective for children who are not hospitalized but are too sick to be managed in the community. This alternative option is the Day Care Approach (DCA). The DCA enables treatment for children with severe pneumonia on a day-care basis in clinics, in the same, safe and effective way as a patient in the hospital, but it remains simple and cost-effective.
The Government of Bangladesh, with support from UN Agency, started a joint programme with ICDDR,B in 2015 to test the Day Care Approach (DCA) as a safe and more affordable alternative to hospital management of severe childhood pneumonia with or without malnutrition. The trial provided solid evidence that children with severe pneumonia can be successfully treated through DCA in both rural and urban settings in Bangladesh by comparing the quality of care and treatment outcomes between hospital (control) and DCA (treatment) group. Based on the positive evidence, the initiative is going to be implemented in two districts and three city corporations. This result will support to generate evidence for further scale up of the DCA for children with severe pneumonia with or without severe malnutrition within the health system of Bangladesh in the country through MNCAH operational plan. To provide the services in the selected day care facilities, the capacity of health service providers must have clear knowledge and skills on management and supervision. Therefore, GoBis taking steps to develop protocols and guidelines for management of severe pneumonia with or without severe malnutrition in line with existing IMCI module. UN Agency will provide technical assistance for development of these protocols and guidelines and training modules for health service providers and health facilities andwill provide technical support for capacity building of service providers and health managers.
In this context, the government has requested UN Agency to provide technical assistance with support of a National Consultant for the development of national guidelines, protocol and training module for the implementation of the Day care for children with severe pneumonia with or without severe malnutrition.
Thus, a National Consultant will be hired to coordinate with NNHP, IMCI and NNS under MoH&FW and city corporations under MOLGRD&C as well as with other partners those who are working for childhood pneumonia and nutrition to develop national guidelines, protocol and training module in line with IMCI & SAM module and to support NNHP, NNS & IMCI for training implementation.
C. Scope of Work:
The National consultant will perform the specific tasks for developing the national guidelines, protocols and related training modules for the Day care for children with severe pneumonia with or without severe malnutrition based on existing IMCI and National guideline for facility-based management of children with SAM protocols and training modules, draft documents and evidence on pneumonia day care in collaboration with MoH&FW/DGHS, City Corporations, UN Agency and other partners. The consultant will also provide technical support to the City Corporations/DGHS/partners to plan, organize and conduct the training on these guidelines and protocols in selected districts and city corporation.
The deliverables will be:
- The national guideline and protocol for implementation of the Day care for children with severe pneumonia with or without severe malnutrition
Time frame:20 days
- A training module on "the Day care for children with severe pneumonia with or without severemalnutrition" based on Review of national protocols on Management of children with severe pneumonia with or without malnutrition-based on IMCI protocol and National guideline for facility-based management of children with SAM
Time frame: 25 days
- Training of service providers in selected facilities of Gazipur, Kishoreganj, Gazipur city corporation, DNCC and DSCC
Time frame: 45 days
The activities of the National Consultant will include but not limited to:
- Review of existing IMCI protocols and National guideline for facility-based management of children with SAMand training modules for management of sever pneumonia, review the documents from the research with ICDDR,B on "Day care management of children with severe pneumonia with or without severe malnutrition"
- To coordinate with the NNHP, NNS, professional bodies, BPA, BNF and related partners as required
- Organizesmall meeting with the NNHP, NNS and respective professional bodies to review the guideline and training modules
- Make a presentation on the final documents to be shared with approval committee.
- Develop and submit the final guideline, protocol and training module to NNHP, NNSand UN Agency
- Coordinate with NNHP&NNS to develop the training plan for the implementing districts and city corporation and support to facilitate TOT and training of service providers in selected facilities

E. Reporting Requirements:
The consultant will be reportable to Health Specialist and will work closely with her for day to day support to develop the national guideline, protocol and training module. S/he will also maintain close liaison with DGHS (NNP&IMCI, NNS and CBHC) and with development partners working for new-born and child health for proper implementation of the activities assigned to him/her.
F. Consultancy Cost:
- 1st payment: BDT 225,000.00 after submission of "The national guideline and protocol"
- 2nd payment: BDT 225,000.00 after submission of Training Module on "the Day care for children with severe pneumonia with or without malnutrition"
- 3rd payment: BDT 225,000.00 after completion of training and submission of final report
- The consultants will get BDT 225,000.00 after submission of each deliverable as a consolidated package which includes lumpsum travel cost within Dhaka, however any additional travel cost to Kishoreganj and Gazipur will be paid on an actual basis based on agreed assignment list
- Computer and communication will be borne by the Consultant
- Stationery and other office equipment will be borne by the Consultant
- Consultant will be responsible to pay all taxes as per government rules and regulations. E-Zone will make statutoty deductions towards income tax as per government rules and regulations.
G. Profile Requirements and Qualification Skills:
- Have MBBS degree with preferably MPH;
- At least 5 years working experience in health-nutrition related project;
- Preference will be for expertise and knowledge on Maternal, Newborn, Child and Adolescent health, including Urban Primary Health Care; nutrition focusing on SAM management;
- Experience of developing guidelines, training packages, tools and job aids on MNCAHN including urban health services;
- Working experience with MoH&FW especially DGHS and City corporation;
- Working in the area of IMCI will be added advantage
- Special priority will be given to those who has experience in working on similar projects with GOB or private sectors
